H.H.C. Group's SIIA Key Takeaways: Turning Cost-Management Opportunities Into Measurable Results

November 02, 2026

Interacting with many of the overflow crowd at the SIIA Annual Conference in Phoenix emphasized a priority we share with clients and industry partners: now is the time to identify and implement practical solutions that can reduce claims costs while maintaining the value of health benefits.

We do not need another survey to tell us that healthcare costs and stop-loss premiums are rising. We continue to hear the same concern from self-funded plans and the organizations that support them: it is time to take action and put active cost management into practice.

SIIA gave us an opportunity to reconnect with longstanding clients, exchange ideas with fellow industry professionals and explore partnerships where real people can deliver real value.

Here are a few of the themes our team came away discussing.

  1. Renewal pressure makes active claims cost management increasingly important.

One message people made clear repeatedly is that rising healthcare and stop-loss costs are forcing plan sponsors and administrators to look more closely at the claims spending driving those increases.

A network discount may be an important first step, but it does not always represent the full savings opportunity. Active cost management goes further by identifying opportunities, determining an appropriate payment strategy, engaging the right provider financial decision-makers and following the claim through to a measurable result.

For high-dollar claims, that may mean reviewing billed services, examining the payment basis and determining whether provider engagement could support a more appropriate result on either an in-network or out-of-network claim.

The question is not simply how much was saved against billed charges. It is whether the final payment is reasonable, supportable and reflective of the opportunities available.

  2. Complex claims require the right combination of expertise.

Our conversations also reinforced that no single type of expertise can answer every question raised by a complex claim.

Financial questions may require medical bill review, repricing, coding expertise or provider negotiation. Clinical questions involving medical necessity, appropriateness or other care decisions require independent clinical review.

That distinction matters. Matching the right expertise to the issue gives plans a stronger basis for determining appropriate payment and deciding where additional intervention may be warranted.

Depending on the claim and service, financial review may also include prepayment or post-payment analysis to identify additional savings opportunities.

  3. Cost-management programs should reflect each client's needs.

People shared their strong preference for solutions that fit the way organizations already operate.

TPAs, health plans, Taft-Hartley funds, group captives and workers' compensation organizations all face different claims challenges, contractual requirements and workflows. A successful cost-management strategy has to account for those realities rather than forcing every client into the same model.

Some of the most productive conversations at SIIA focused on identifying gaps, defining appropriate referral criteria and determining how specialized services could fit into existing operations.

The goal is to strengthen the existing healthcare ecosystem without adding unnecessary disruption or administrative burden.

H.H.C. Group offers an initial consultation at no charge to explore those needs and discuss a tailored approach.

  4. Strong partnerships help move opportunities into action.

One of the biggest benefits of SIIA is the opportunity to strengthen relationships across the self-insurance community.

TPAs, consultants, stop-loss partners and specialized service providers each bring different expertise to the table. Those relationships are most valuable when everyone understands when additional expertise is needed, who has authority to act and how to keep a difficult claim moving toward resolution.

That is also where execution matters. Identifying an opportunity is only the first step. Delivering a result requires the right people, the right strategy and persistent follow-through.

Moving From Discussion to Action

We left SIIA encouraged by the conversations we had and by the industry's focus on practical, measurable solutions.

For us, the takeaway is simple: rising renewal costs make it even more important to look beyond the headline numbers and focus on the claims-level decisions plans can still influence.

H.H.C. Group helps clients move from identification to action through specialized financial expertise, intelligent technology and strategic provider engagement, with independent clinical expertise available when objective review is required.
